Probably because it is a legit outcome of plastic surgery that plastic surgeons discuss with their clients, that there is literature on and that there are procedures described for to deal with it. Make assumptions about whatever sinister reason behind it, but pretending there aren't other, way more reasonable explanations, is outlandish and detached from reality.
