When I was much younger I used to work at Equity meats. At the time they were the only places on earth that made the patties. They are 100% beef and are laser tested. Most of the people in North Baltimore Ohio have worked for the company their whole lives. Even though now it is under a different name the process is the same. Check it out. Every single batch is tested for consistency. While their "diet" menu items may not be all beef the normal patties are.

Seriously it is just a flash frozen piece of meat. Nothing is added. They grind their own sides of beef. Now as for what is put on it, that is a completely different story.
