Yes health care facilities are required to give you a hard copy of what you are signing when REQUESTED. Read the hard copy, cross out and initial and date anything you do not agree to. Sign and date each page. Also write on the copy that you want this placed in your medical record Then have them make you a copy for your records. 😇
