Not saying that Betty wasn't a man but if he/she is actually a woman, let's say he/she is, then that could be thyroid goiter. Women are more prone to it. You can get a good look at women with this issue online. If it is goiter the swollen areas can be small like what is on the neck in the picture and in many instances huge. Seems that if it were an adam's apple it would be more of a large hard lump not a swollen filled something. But knowing the history of most of the actors she is probably a he. But men have thyroid too and have the same issues. So perhaps it is not a swollen adam's apple but goiter whether a man or woman. Betty wasn't well in this picture nor mentally at any time as I remember the coat hangers her daughter spoke of.
