This appears to be a movie within a movie. "Director" was wearing a mask. He would not have appeared in this hospital scene, which is purportedly being filmed to show the people how bad the pandemic is which would make them self-isolate.

So, if that is the case, why is HE wearing a mask? Because he is himself an "actor" and people would know who he is
