You just call them. they were nice to me. I had to do it 2 years in a row because of financial problems.
I spoke to them to get a total of how much it was the first year. It was almost $20,000. They broke it down into monthly payments for me. They were nice to me. You need to keep track of your payments so they don't get lost. I made payments through eftps which is a website that pays to the IRS. I did it because I could keep track of it, and when they paid the IRS it was documented, because I have had the IRS lose checks that I sent and then I have to send it again and it is considered late even though they lost the check.

EFTPS website is free. Call the IRS early in the morning and maybe ask how much to pay and negotiate with them. You can set up the payment plan online but call first and take lots of notes. They can also help you set it up.
If you get a shitty person on the phone then hang up and call back. Every once in a while I would get an asshole, but most of the time I got someone nice.
